On March 13, 2018, the Order of the Ministry of Health of Ukraine No. 157 of January 26, 2018 came into force, canceling a number of forms of primary accounting documentation. Yes, form No. 025-3/o "Student's medical card" is also canceled. From now on, all students will have a regular outpatient card.
The Ministry of Health of Ukraine reminds that with the launch of the reform in cities, students studying there will be able to choose any family doctor or therapist at the place of actual residence, sign a declaration with him and receive all services along with any necessary health certificates.
Student polyclinics that exist now will compete for patients on general terms.
So, according to the order, the following forms also lose their validity:
No. 025-2/o "Coupon for registration of final (specified) diagnoses";
No. 030/o "Dispensary supervision control card";
No. 025-4/o "Coupon for an appointment with a doctor";
No. 025-6/o "Ambulatory Patient Coupon";
No. 026/o "Medical card of a child (for preschool and general educational institutions)";
No. 031/o "Book of records of doctors' home calls
